Today, bluevelvetrestaurant.com would like to introduce to you Community Health Nursing – Community Assessment and Community Health Plan. Following along are instructions in the video below:
In this video. Im gonna talk a little bit about doing a community assessment. Assessment.
As well as developing a community health plan. So when we are doing a assessment. We are assessing both the people in the community as well as the environment.
So when were assessing the people in the community. We want to take into consideration things such as demographics biological factors such as race age and gender as well as social factors such as income and education and then cultural factors such as history religion and customs when were assessing the environment. We want to consider both physical factors as well as environmental factors.
So physical factors can include things.
Such as geography. Housing and location of healthcare services. And then environmental factors.
Can include things. Such as climate pollutants as well as topography so when we are collecting data about our community. We can do this through a number of ways so im going to run through some of these ways.
So you can be familiar with them. So we can do informant interviews. This is where we have direct conversations with members of the community.
We can also do a community forum.
Which is where we have a public meeting where we invite members of the community to attend the meeting sometimes participation can be an issue with this and then we can collect secondary data. So when i say secondary data. I mean that this data actually already exists so we can go look at statistics or like health records to gain more information so these records and these statistics already exists.
Which is why we kind of call them secondary data. And then we can do participant observation. Where we basically observe community activities.
And then we can do whats called a windshield survey. Which is literally where youre driving around in your car and looking out your windshield at the people and the places in the community and making observations and were gonna talk a little bit more about that in a minute. We can also do focus groups.
Which is where we meet with a representative sample of the community and then we can do surveys.
Which is where we kind of send out a survey in written format. It can be very expensive to do this and a lot of times. The response isnt very good so sometimes.
This isnt the best option in terms of getting data on our community. So lets talk more about that windshield survey. So were were driving around the streets of the community and were making observations.
So what are we observing for we want to observe for the kind of general appearance of the people in the community we want to look for signs of violence signs of mental illness like drug abuse we also want to take a look at the quality of housing as well as access to health care facilities. We want to look at the type and availability of grocery stores as well as public transportation and we want to see about schools parks and churches in the community. So when we are ready to go ahead and develop our community health plan.
There are different steps that we will go through the first step is pre planning.
So this is where were really just brainstorming ideas and then we do our assessment. So this is where we kind of collect data through one of the means that we just talked about so. Like the windshield survey or community forums.
Etc. And then were gonna do our diagnosis. Step.
This is where we analyze the collected data to determine the health needs of the community and we set priorities like whats going to be most important for the community and then we do our planning step. This is where we plan interventions we establish goals we identify responsibilities and we develop a budget for the intervention and then the next step is implementation. So this is where we execute our plan and then of course.
We want to do an evaluation next. So evaluation is the last step and this is where we determine the effectiveness of our intervention. So hopefully.
